OUTPATIENT/PCP BEHAVIORAL HEALTH

The partial care program is geared to adults who are transitioning from hospital to the community and are either diagnosed with severe and persistent mental illness or have co-occurring mental health conditions/substance abuse.

Outpatient Behavioral Health Centers are effective in reducing or eliminating the incidence of hospitalization. Participants are empowered to regain and improve stability and self-sufficiency, leading to a more independent and productive lifestyle.
